Are you sure you are modifying the class you want? PHM is the Warrior class. check out the prefixes at the beginning of my post to tell if you a modding the right one.
After you find out what is the prefix of your class (see my post), use this:

Yes, if you remove the Red Underwear line, it will go back to normal and show your normal underwear xD
I'm still trying to figure it out how this swap to the red underwear works, I'm just as new as you to this thing, I basically just try and test to see what happens.
Try moving your Black Desert Online folder to your desktop, and then run the .bat you want.
Running the .bat isn't necessary, if you extracted the BDNudePlusPatcher(v1).zip to you Black Desert Online\ folder, all you have to do is delete your PAD00001.PAZ from your \PAZ\ folder (not permanently, you might need it later) and then run the launcher.
It works fine with the NA/EU version, even after the last update.
