You need bought underwear from my experience, they change just fine if your inner is bought from the cash shop... Otherwise it doesnt work at all with standard ones, Im pretty sure the standard inners are just apart of the texture
I do have bought underwear and armor but it is possible without putting the texture on the underwear right?
Did you install Resorep, textures and Armor Swap Essentials?

Yes i have installed resorep, armor swap and skins and its working fine if im wearing something as example the kibelius armor only there is no underwear. but as soon as i undress everything i end up with standart armor/underwear.

From experience, there are some errors in the outfits, I would advise you to test all the codes, or swap the code from the Yuldo to the see if they accidentally switched the codes.

EDIT: got it! it's 0067!

However, the hair clips through the hat and I tried to add hair in the partcombinationdesc to no avail.

Does anyone know why the witches pirate costume is doing this? I want it to look normal like everyone elses

You can try removing these two files from your Resorep "modded" folder:
3534311588.dds
3545795712.dds
I've not tested this, so I'd recommend you only move those files somewhere (rather than delete), so you can put them back if it doesn't do what you want.

I've got the same bug, i don't play a witch so it doesn't bother me, however i do not have those files in my modded resorep folder to remove

Thanks Nicky i'll check if those files are in my resorep folder tomorrow (on my laptop atm) and i'll report back the results ^_^

Great mod, fantastic work!

I do have a question however. Is there any way to swap a damaged model with an undamaged one. Basically, I would like to completely negate the damaged effect from my equipment. I've tried replacing the dm.PAC file with the standard .PAC file in the damaged conditional thread of the xml with no success. Any help or advice would be appreciated.

heya nicky I just checked my resorep folder and I don't have either of those two files, do you have any other ideas? or its unmoded files I can add to resorep?
